Your responsibilities include:

  • Assessing the health needs of individuals, families, and communities through comprehensive health assessments and screenings.
  • Developing and implementing individualized care plans that address specific health concerns and promote overall well-being.
  • Providing health counselling and education to patients and their families on various topics such as healthy lifestyle choices, disease prevention, and management of chronic conditions.
  • Conducting health promotion activities, including community workshops, presentations, and health campaigns to raise awareness and empower individuals and groups to make informed decisions about their health.
  • Collaborating with community organizations, healthcare providers, and social services to facilitate access to resources and support networks for patients.

  • Monitoring and evaluating patient outcomes, tracking progress, and adjusting care plans as needed to ensure optimal health outcomes.
  • Participating in interdisciplinary healthcare teams to coordinate and deliver comprehensive care to individuals with complex health needs.
  • Providing support and assistance in navigating the healthcare system, including referrals to specialized healthcare professionals and community services.
  • Documenting patient interactions, interventions, and outcomes accurately and maintaining thorough and up-to-date records.
